Together with T-Systems Austria, Tirol Kliniken GmbH is modernizing its SAP-based IT and making it fit for the future. As a part of this initiative, the company is migrating its SAP landscape to the RISE-certified T Cloud Private from T-Systems and is also introducing T-Systems Solution for HealthCare (TSHC) as the successor solution for SAP IS-H—as the basis for excellent medical care.
Tirol Kliniken GmbH is the largest and most diverse healthcare company in western Austria. It employs over 7,200 full-time staff and forms the backbone of medical care in Tirol. The clinic group includes three state hospitals, a care facility, a school, and 11 subsidiaries. Almost 2,300 state-authorized beds are available at the university hospitals in Innsbruck, the state hospital in Hall, and the state hospital of Hochzirl-Natters. Every year, the hospitals admit around 95,000 patients as (partial) inpatients. A total of over 1.7 million cases are treated in the outpatient clinics.
The industry standard is used for financial processes in healthcare operations: SAP. All central data converges in this ERP system. Despite the almost universal use of SAP IS-H worldwide and especially in Austria—including in Tirol Kliniken—SAP has discontinued further development and support for the solution. After decades of experience in the maintenance and further development of IS-H on behalf of SAP, T-Systems launched the TSHC in 2025, providing a future-proof successor solution. The project is being implemented with ATSP, the partner of T-Systems Austria, which ensures a largely automatic, personnel-friendly, and secure conversion to the TSHC.
In 2025, Tirol Kliniken discussed two key SAP trends: on the one hand, the switch to SAP Cloud ERP within RISE with SAP and, on the other, how a migration to S/4HANA should take place. Previously, it operated SAP systems on-premises in a service provider's datacenter. The latter announced the relocation of its operations to outside the European Union. Tirol Kliniken did not want to go down this route and wanted to ensure operations in Austria.
In combination with the ongoing RISE with SAP and SAP S/4HANA discussions, the clinic group decided to fundamentally reassess its SAP engagement and reposition itself. An important aspect of this was the search for a new operating platform. However, SAP Cloud ERP favors US hyperscaler platforms as standard. Last but not least, there was also the question of what impact the move to the cloud would have on the upcoming IS-H replacement. Tirol Kliniken approached one of the most experienced SAP partners in Austria: T-Systems Austria.
Together with T-Systems Austria and ATSP, Tirol Kliniken analyzed the SAP Cloud ERP offerings and evaluated options for a viable future operating concept. As a global premium supplier for RISE with SAP, T-Systems brought transparency to possible development paths with its consulting services. T Cloud Private is certified as a platform for RISE with SAP and enables fully sovereign operation of the SAP landscape in Austria.
In 2026, T-Systems will migrate the on-premise SAP systems to T Cloud Private in the datacenter in Vienna. Established procedures will be used to minimize the migration risk and ensure minimal downtimes. From October 2026, T-Systems will operate the clinic's SAP systems in its own cloud datacenter. At the same time, T-Systems is replacing SAP IS-H with TSHC. The patient billing solution runs as an SAP application under SAP S/4HANA.

The use of TSHC means continuity for employees. It maintains the familiar working environment for patient billing and ensures continuous support and the further development of the billing system. It also offers a successor solution for IS-H that largely retains the proven functional depth of SAP IS-H, including all Austrian requirements. At the same time, the solution was developed further in a targeted manner: frequently used functions were optimized, while rarely required functions were consistently reduced. The result is a leaner, more powerful feature set with a high recognition value, stable data structures, and almost identical user guidance.
This makes the changeover efficient and resource-saving—with minimal training for users and IT. The know-how built up by IT experts over the years will also be fully retained during the changeover and can continue to be used seamlessly. New functionalities are consistently implemented using modern technology and a contemporary look and feel. All in all, Tirol Kliniken is positioning itself perfectly for the future: in addition to SAP operations, T Cloud Private also offers the clinic group prospects for the efficient operation of non-SAP applications. The IT backbone provides clinics with a strong basis for the continuation of excellent patient care.
